HOW TO MAKE A TAX DEDUCTIBLE DONATION

Your tax deductible charitable gift provides quality care for indigent patients or those without a hospice insurance benefit, provides grief support for families and friends of hospice patients and those in our service community, and pays for quality care not fully reimbursed under the per day hospice benefit.

Our pledge to you as a donor is, first and foremost, to be responsible stewards of your financial gifts and to manage such resources with integrity and transparency.We respect your privacy and will not share your information with other organizations.Also, we value every gift, large or small, to support the mission of Hospice Care of Middletown.

TO SEND A DONATION BY MAIL

If you prefer to mail your donation, please make checks payable to Hospice Care of Middletown, and mail to Hospice Care of Middletown, 4418 Lewis Street, Suite B, Middletown, OH, 45044. If your gift is in memory of or in honor of someone, please include a notation with the check, and the name and address of the person to whom you’d like an acknowledgment sent.
